Customer Survey Informed Consent Policy

This Customer Survey Informed Consent Policy (“Policy") governs the collection, use, and disclosure of survey responses provided by you ("Respondent") to Magneto & Diesel Injector Service, Inc. dba M&D Distributors ("Company" or “M&D”). By completing this survey, you acknowledge that you have read, understood, and agree to the terms and conditions set forth in this Clause.

1.  Survey Participation and Data Collection

1.1 The Company is conducting this survey to gather feedback about our customers' experience. Your participation is voluntary, and you may choose to discontinue the survey at any time.

1.2 The information collected through this survey may include your opinions, preferences, experiences, and other feedback related to our products and/or services.

2.  Social Media Usage Authorization

2.1 By answering the following question affirmatively, you grant the Company permission to use your survey responses, including any comments, ratings, or feedback, on the Company's social media accounts:

Survey Question: "Do you consent to M&D sharing your survey responses, feedback, and comments on our social media accounts? Your personal identifying information will not be shared unless specifically authorized below."

2.2 If you select "Yes" above, you acknowledge and agree that:

1.  The Company may use, reproduce, publish, and display your survey responses on various social media platforms, including but not limited to Facebook, Twitter, Instagram, LinkedIn, and other current or future social media platforms.

2.  Your responses may be used in whole or in part, and may be edited for clarity, length, or formatting purposes while maintaining the substance of your feedback.

3.  Unless specifically authorized below, your personal identifying information will not be included when sharing your responses.

3.  Optional Personal Information Disclosure

3.1 If you wish to allow the Company to attribute your feedback to you on social media, please complete the following optional authorization:

4.  Ownership and Intellectual Property

4.1 By providing your survey responses and consenting to their use on social media, you grant the Company a worldwide, royalty-free, perpetual, irrevocable, non-exclusive license to use your responses for marketing, promotional, and informational purposes.

4.2 You represent and warrant that your survey responses are your own original thoughts and do not infringe upon any third-party intellectual property rights, privacy rights, publicity rights, or other personal or proprietary rights.

5.  Limitations and Restrictions

5.1 The Company shall not use your survey responses in a manner that is discriminatory, threatening, incendiary, libelous, obscene, harassing, lewd, vulgar, slanderous, or untrue.

5.2 You may revoke your consent for future use of your survey responses on social media by contacting operations@mddistributors.com with your request. The Company will make reasonable efforts to remove your responses from social media accounts within 72 hours of receiving your request, though previously published content may not be retrievable from all platforms.

6.  Governing Law

6.1 This Clause shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law provisions.
